Patient Access Partnership Program
Carlsmed is patient obsessed and aims to help patients obtain access to the benefits associated with personalized spine surgery with aprevo®. We have partnered with JDL Access to assist patients and their surgeons in obtaining insurance company prior authorization. Before any assistance can be provided, a signed Patient Authorization and Consent form must be received from the provider.

Patient Transportation Reimbursement
If your doctor has ordered new imaging, it important that you complete this as soon as possible. To help defray your travel costs, Carlsmed will reimburse your transportation expenses to and from the imaging facility. Thank you for taking this important step to participate in your healthcare.
